Standard Optical Mark Read Answer Sheets

The powerful combination of the convenient OMR sheets and computer technology offers timesaving applications in data collection and data entry for surveys and research projects. A variety of standard optical mark read answer sheets are available from the Office of Measurement Services for Course Exam use.

They include:

The standard answer sheet (No. 402) contains 216 5-choice items with responses numbered (1-5) and lettered (A-E). It is available in two colors, red and orange.
An alternate form (No. 401) has responses lettered (A-E), with the first two responses also marked for true-false responses. This is available in orange.
Answer sheet No. 507 contains 144 10-choice items with responses numbered (0-9) and lettered (A-J).

All sheets are complete with grids for name, identification number, special codes, including course and section numbers, sex, type of grade (A-F or P-N), class, test form and graduate/undergraduate status.

Answer sheets may only be obtained at the OMS office. No other locations will be distributing answer sheets.

The cost of answer sheets is $.10 each and $35.00 per box of 500 for U of M clients.
